Patient Eligibility

Patient’s social worker must have completed and submitted a Jar of heARTS “Patient membership application”

Patient must be 21 years old or younger.

Patient must have a cancer diagnoses.

Patient must be in active treatment, palliative care or in remission within 6 months of request.

Patient must NOT be receiving a Birthday or end of treatment celebration from another local organization.

Request must be submitted 2 weeks in advance by Child Life department or Social Worker.
